Yes, always. If the work is creative and accomplished than it's fair that the people earn some money with it. If not the work would't sell anyway.
I give permission for prints after request and I ususally don't refuse.
On occasion. It depends on the quality of work that has been done with my stock.
No, I strictly forbid it. I don't want people making money from my work.

yes I do.
But there is another point that should be consider with "free for commercial use" Stock. Yes, many allow prints... but why just on dA ? even more important question... why small resolution pictures, brushes, etc. then : I don't see the point by allowing prints but not giving high quality stock (high resolution) ! like if people could really make something decent from it !
